The Mantra sounds in the following way, I have checked the Sanskrit edition of "Shree Varaha-Purana":
शिशिरो भवान् धातरिमं लोकनाथ हिमम् दुस्तरं दुष्प्रवेशं कालं संसारान्मां तारयेमं धर्ता त्रिलोकनाथ ।
shishiro bhavAn dhAtarimaM lokanAtha himam dustaraM duShpraveshaM kAlaM saMsArAnmAM tArayemaM dhartA trilokanAtha |
